Minor Waiver of Release and Liability

By signing below, I, as the parent or legal guardian of the minor participant, agree to the following terms and conditions related to their participation in RS Move, LLC’s (“RS Move”) personal training sessions and activities (collectively referred to as “Sessions”).

Representation of Health
I represent and warrant that the minor participant is in good physical health and does not suffer from any medical condition that would limit their participation in Sessions. I understand that it is my responsibility to consult with a physician prior to and regarding the minor’s participation in Sessions.

Acknowledgment of Risks
I acknowledge that there are inherent risks associated with the activities offered by RS Move. I agree that the minor will follow all instructions provided to ensure their safety during participation in Sessions.

Waiver and Release of Liability
I hereby WAIVE AND RELEASE RS Move, LLC, its owner, employees, and trainers from any claim, demand, or cause of action of any kind resulting from or related to the minor’s participation in Sessions. I acknowledge and accept full responsibility for any and all risks, injuries, or damages, known or unknown, which might occur as a result of the minor’s participation.

Photo and Video Release
I acknowledge and agree that photographs and/or video may be taken of the minor during their participation in Sessions or at RS Move facilities. I grant RS Move, LLC, its legal representatives, successors, and assigns, the irrevocable and unrestricted right to use the minor’s likeness in any manner or medium for editorial, trade, advertising, commercial, or other business purposes, including but not limited to, litigation. I waive the right to inspect or approve the finished product containing the minor’s likeness and waive any right to payment or compensation. I release RS Move, LLC from all claims and liabilities related to the use of such media.

Liability for Personal Property
I acknowledge that RS Move, LLC shall not be liable for any personal property that is damaged, lost, or stolen while on or around the facility, including vehicles, their contents, or property left in lockers. I accept full liability for any damage caused by the minor or their guests to the facility, equipment, furniture, or fixtures.

By signing below, I confirm that I have read and fully understand the above release and waiver of liability. I agree to the terms voluntarily and acknowledge that I am legally authorized to sign on behalf of the minor participant.
